Hallux valgus (bunion) treatment with doc. MUDr. Rastislav Hromádka, Ph.D., a leading Czech orthopaedist

Bunions are annoying you for a long time already, but you are afraid that surgery will shut you down for a while? Our podiatrists can treat this problem gently and fast. How exactly? We have asked doc. MUDr. Rastislav Hromádka, a leading Czech expert in this medical field. Find out more about the procedures which can give you a relief.

1. You offer a mini-invasive treatment as a one day surgery. How is this procedure done?

We strive to provide patients with the best care. The outcome of the operation depends not only on the quality of the technical performance, but also on subsequent medical assistance. The procedure is followed by a hospitalisation, so altogether you don’t spend more than 24 hours in the clinic. The patient usually comes in the morning on the day of the procedure and the next morning is released into home care.

The mini-invasive procedure itself is a combination of modern osteotomy techniques, where a rotary cutter and titanium screws are used to cut the bones. These can be introduced into the patient’s body through small punctures (under the X-ray control), so we do not disturb large areas of soft tissue. It also does not impair the vascular supply of the bone, which leads to a better course of bone healing.

2. You were the first surgeon in the Czech Republic to perform such procedure in 2015. What progress has been made since then? What are the latest trends?

I need to clarify this. I have used this technique for an orthopaedic surgery here, but these osteotomies are already commonly used in dental surgery these days. Changes happen all the time. Recent developments can be seen in the shape change of rotary cutters for bone cutting, thus being more accurate and sharper, thanks to their new shape.

3. You have developed the first Czech prosthesis of the metatarsophalang joint. Can you tell us more about it?

Yes, years ago I designed the shape of the first Czech prosthesis. The prosthesis was a result of work of a team of surgeons and technicians. It is used to correct other common deformity of the MTP joint, called hallux rigidus (stiff toe). This prosthesis is not used for treating the bunions (hallux valgus) cases.

4. Many people have problems with their feet due to modern lifestyle. Can you advise on some basic rules for keeping healthy feet?

Proper muscle tension and appropriate load on the calf muscles are important. If the calf muscles are more strained than they should, walking will cause overload the forefoot and increases the pressure on the toes. Wearing high heels is not as inappropriate as it was claimed, but it increases the tension in the calf muscle, which can cause harm. Therefore, exercises focused on stretching the calf muscles are important. I also recommend good foot care, massages, stretching leg joints and also warm baths, which allow better blood circulation and regeneration. Loose joints, if deformity is already present, also hurt less.
